  • Personal Information: I may collect personally identifiable information, such as your name, email address, and phone number, only when voluntarily submitted by you or a referring health provider.

  • Sensitive Information: I may collect this information when voluntarily submitted by you or a referring health care provider. This can include but is not limited to ethnicity, gender, sexuality, and health. 

    • All information will be used by us only:

    • For the primary purpose for which it was obtained

    • For a secondary purpose that is directly related to the primary purpose

    • With your consent; or where required or authorised by law.

  • Non-Personal Information: I may automatically collect non-personal information, such as your browser type, operating system, and IP address, to enhance your experience on the website.

 

Purposes of collection, use and disclosure

I collect, use and disclose personal information for the purpose of conducting operations and providing services to you and communicating with health providers with your consent.

I collect non identifiable information to improve your experience on the website.

Your Personal Information may be disclosed in a number of circumstances including the following:

How I Protect Your Information

I implement security measures to maintain the safety of your personal information when you enter, or submit your personal information.

When your Personal Information is no longer needed for the purpose for which it was obtained, I will take reasonable steps to destroy or permanently de-identify your Personal Information. Your health records will be retained for a minimum of 7 years.

AE Clinical Psychology uses a secure practice software and messaging management system that is ISO 27001 certified (an international standard for information security management systems) and Secure Socket Layer (SSL) technology on web inquiries.

Browsing information that is non-personally identifiable from the website is collected. This information is collected for the sole purpose of measuring visits to the site and making improvements to the content.

I will not charge any fee for your access request but may charge an administrative fee for providing a copy of your Personal Information.

To protect your Personal Information, I may require identification from you before releasing the requested information.

It is important that your Personal Information is up to date. I will take reasonable steps to make sure that your Personal Information is accurate, complete, and up-to-date. If you find that the information I have is not up to date or is inaccurate, please advise as soon as practicable I can update records and ensure continued quality services to you.
